If you have experienced or witnessed an incident related to our services, please use the form below to report it confidentially. Your report helps us improve our services and ensure a safe, respectful, and supportive environment for everyone.

What to Report?

You can use this form to report:

  • Accidents, injuries, or medical emergencies
  • Abuse, neglect, or exploitation
  • Property damage or hazards
  • Breaches of participant rights
  • Any other safety or well-being concerns

Your Privacy Matters

All reports are treated with strict confidentiality, and we will handle your concerns with care and sensitivity. Your information will only be used for investigation and resolution purposes, in line with the Privacy Act 1988 and NDIS Code of Conduct.

How to Report an Incident?

  1. Complete the Form – Provide details of the incident, including date, location, and persons involved.
  2. Submit Your Report – Your submission will be securely received by our compliance team.
  3. Investigation & Action – We will review your report and take appropriate action to address the issue.
  4. Follow-Up & Support – If required, we will keep you informed of any necessary outcomes or support options.

More Information

Incidents are “an act, omission, event, or circumstance that causes harm, damage or loss, or could reasonably be expected to cause harm, during or in connection with an arranged service between Accessible Spirit Care's platform users”.

Incidents differ from complaints, so if you wish to make a complaint or provide any feedback to Accessible Spirit Care, please fill out this form instead.

We aim to review any incidents within 48 business hours and may contact you to seek additional information as part of that process.

If a registered disability service provider or an approved home care provider is connected to the participant or consumer’s account, Accessible Spirit Care will forward the incident details to them for review and investigation.
